Hop 3 Timing Out on Game Servers - Need Help Finding Out Why

Hello, I'm a long time lurker and first time poster.

So I'm trying to determine exactly what this means so I can communicate with my ISP and further tweak my internet setup to be more reliable for online gaming. My connection is unreliable during gaming sessions about 40% of the time and it's very difficult to play for a long time without experiencing lag spikes. My ping, when not experiencing spikes, is usually around 20 to 60.

So a tracert to any Battlefield 4 game server always looks something like this:

Going to the third hop's IP address in my browser brings me to a page asking me to input a username and password and is called "Broadband Bonding Network Appliance." I think this might possibly be my modem? If so does that mean a connection is being blocked by my modem to gaming servers? I'm just not sure what to make of it but I feel like it might help if I can get that to stop timing out.
